It seems almost fitting that on the 5th of November, Guy Fawkes night, that I would spend the day working out different ways to blow stuff up.
Today was the first day performing a Process Hazard Analysis (PHA) of the plant I used to look after as an operations engineer. It was interesting the difference of performing a PHA in a plant that has been operating for many decades, compared to a Hazop of a hypothetical plant that has not yet been built.
But it was still a surprise at how many ways we came up with that could cause a problem. And we are still only looking at the feed system.
